The Kostiuk ALS Care Grant
The Kostiuk ALS Care Grant Program is a two cycle reimbursement program that assists families with expenses that are not traditionally covered by private insurance, Medicaid, or Medicare.
The Kostiuk ALS Care Grant Program is funded by public and private foundations. There are two separate grant cycles per year. The maximum reimbursement per cycle is $1,000. This provides the opportunity for a total reimbursement of up to $2,000 per year of qualified ALS related expenses.
The first grant cycle for 2023 has closed. The second cycle application will open in July 2023.

How to Apply
At the beginning of each new cycle, the Georgia Chapter will send out a postcard to all individuals with ALS that are registered with the Georgia Chapter inviting you to apply.

Reimbursement Requests
Applicants will be notified within two weeks after the closing of the application period. Individuals awarded the Kostiuk ALS Care Grant will need to complete a billing statement form for reimbursement. If you are requesting reimbursement for services provided by a third-party, such as an in-home care provider, please complete page 2 on the Grant Services Receipt Form. Please review the reimbursement chart before submitting your request. Instructions and specific time-frames for submitting reimbursements will be provided with the approval letter.

One billing statement form per cycle, along with the corresponding receipts, is to be submitted. Once the reimbursement paperwork has been received, we will issue and mail you a check within 2 - 3 weeks of receipt.
